Job summary

We are looking for an enthusiastic and experienced Advanced Nurse Practitioner (ANP) to join our welcoming GP practice for 4 days per week, including Mondays. You will work as part of a friendly, supportive multidisciplinary team, providing high-quality, patient-centred care to patients with a wide range of acute and chronic health needs.

Main duties of the job

You will provide autonomous assessment, diagnosis and management of patients presenting with a range of acute and chronic conditions within a GP practice. Patient contacts are generated through the practice's online triage system, with requests clinically assessed by a GP and allocated to you within an agreed number of daily contacts. You will manage your allocated patients, request investigations, prescribe where appropriately qualified, make referrals when required, and maintain accurate clinical records. Working closely with GPs and the wider multidisciplinary team, you will contribute to the delivery of safe, effective, and patient-centred care while supporting the ongoing development of practice services.

Job responsibilities

Key Responsibilities

Clinical Duties

  • Undertake face-to-face, telephone, and video consultations
  • Perform holistic clinical assessments and examinations
  • Diagnose and manage acute presentations within scope of practice
  • Carry out home visits for housebound or vulnerable patients
  • Develop and implement management plans
  • Escalate or refer patients to GPs or secondary care when appropriate
  • Support continuity of care within the practice

Urgent Care & Triage

  • Manage same-day urgent appointments
  • Triage patients based on clinical need and risk
  • Support reduction of GP workload through autonomous practice

Prescribing & Medicines Management (if qualified)

  • Prescribe safely as an Independent Prescriber (if applicable)
  • Undertake medication reviews
  • Work within national and local prescribing guidelines

Multidisciplinary Working

  • Work collaboratively with GPs, nurses, pharmacists, and wider PCN teams
  • Participate in MDT discussions and care planning
  • Contribute to integrated patient care across services

Governance & Quality

  • Maintain accurate, timely clinical records
  • Participate in audits, quality improvement, and significant event reviews
  • Comply with safeguarding policies and clinical governance frameworks

Professional Development

  • Maintain registration with the relevant professional body
  • Engage in CPD and ongoing training relevant to primary care
  • Participate in appraisal and revalidation

Person specification

Experience

Essential

  • Registered Advanced Nurse Practitioner
  • Experience in urgent or emergency care
  • Strong clinical assessment and decision-making skills
  • Ability to work independently within scope
  • Excellent communication skills

Desirable

  • Experience in primary care or GP setting
  • Independent Prescriber qualification
  • Familiarity with EMIS
  • Experience managing long-term conditions
