Job summary

We are seeking a highly motivated and experienced Advanced Nurse Practitioner (ANP) to join our friendly and innovative GP practice team. The successful candidate will play a key role in delivering high-quality patient-centred care, with a particular focus on undertaking ward rounds within our care home settings and supporting the management of patients with complex and long-term health needs.

Working autonomously and alongside GPs, nurses, pharmacists, and the wider multidisciplinary team, you will assess, diagnose, treat, prescribe, and manage patients, ensuring continuity of care and reducing unnecessary hospital admissions.

Main duties of the job

  • Undertake regular ward rounds within designated care homes.
  • Assess, diagnose, and manage acute and chronic health conditions.
  • Independently prescribe medication in line with clinical guidelines.
  • Conduct comprehensive clinical assessments and formulate management plans.
  • Manage unplanned and urgent care presentations.
  • Support proactive management of frail and vulnerable patients.
  • Work collaboratively with care home staff, community teams, social care, and secondary care providers.
  • Provide clinical leadership and education to colleagues where appropriate.
  • Maintain accurate and contemporaneous clinical records.
  • Participate in clinical governance, audit, and quality improvement activities.

Person specification

Qualifications

Essential

  • Registered Nurse with current NMC registration.
  • Master's level qualification in Advanced Clinical Practice or equivalent.
  • Independent Non-Medical Prescriber qualification.
  • Significant experience working as an Advanced Nurse Practitioner in Primary Care, Community Care, or Urgent Care settings.
  • Excellent assessment, diagnostic, and clinical decision-making skills.
  • Ability to work autonomously within professional scope of practice.
  • Strong communication and multidisciplinary team-working skills.
  • Full UK driving licence and access to a vehicle.

Desirable

  • Experience of working with care home residents and frailty services.
  • Qualification or experience in geriatric medicine or long-term condition management.
  • Experience of clinical leadership and service development.
