Job summary
Wearside Medical Practice is a well organised, forward thinking and friendly Practice with an approximate list size of 7900 patients. We are a busy Practice situated in the heart of Sunderland. We are looking forward to recruiting and welcoming an Advanced Nurse Practitioner to expand and enhance our team, working Part -time (Monday and Friday )

Main duties of the job
Working as an independent prescriber who will be able to diagnose and treat patients of all ages presenting with acute problems, via face to face and telephone consultations.
There will be a requirement to carry out home visits for our housebound patients.
You will be responsible for clinic generated administration (tasks, path lab results etc.)
Helping to reach QOF targets
Participating in clinical audits
About us
We currently have 3 GP Partners, 2 salaried GP's, 2 Advanced Nurse Practitioners supported by 2 Practice Nurses and 2 Health Care Assistants and a full complement of administrative staff.
We were rated overall Good by CQC in June 2022.
The Practice maintains a very high QOF achievement each year.
The Practice uses EMIS Web supported by GP TeamNet
Part of care home alignment scheme the Practice holds weekly MDT meetings to improve care of patients with complex needs.
The Practice holds regular formal clinical and nursing educational/MDT meetings.
Job responsibilities
The Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The post holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Advanced Nurse Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The post holder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.

Qualifications
Essential
- Registered General Nurse
- Recognised ANP qualification at Masters level or equivalent
- Independent Nurse Prescriber
- Knowledge of National Standards that inform Practice e.g. National Service Frameworks, NICE guidelines
Desirable
- Evidence of working autonomously and as part of a team
- Ability to work flexible hours when required
