We're looking for a passionate and motivated Band 5 Occupational Therapist to join our supportive and forward-thinking Rapid Response and Intermediate Care Team, based at Sheppey Community Hospital. Main duties of the job

As a Band 5 Occupational Therapist at HCRG Care Group, you will undertake structured rotations across Intermediate Care, Rapid Response, and the Inpatient Rehabilitation Unit. You'll complete comprehensive occupational therapy assessments and develop person-centred treatment plans. You will deliver evidence-based interventions that support independence, rehabilitation, and recovery. Working collaboratively with patients, families, carers, and other healthcare professionals, you will strive to achieve positive outcomes for all involved. Additional responsibilities include conducting follow-up reviews, monitoring progress, assessing equipment and environmental needs for safe discharge, and providing guidance to Rehabilitation Assistants and Support Workers. Maintaining accurate clinical records, participating in multidisciplinary meetings, and contributing to service development initiatives are also key parts of the role. This service operates 7 days a week between 8:00am and 8:00pm, including weekend and bank holiday working on a rota basis.

Main Responsibilities

As a Band 5 Occupational Therapist, you will:

  • Undertake structured rotations across Intermediate Care, Rapid Response and the Inpatient Rehabilitation Unit.
  • Complete comprehensive occupational therapy assessments and develop person-centred treatment plans.
  • Deliver evidence-based interventions that support independence, rehabilitation and recovery.
  • Work collaboratively with patients, families, carers and other healthcare professionals to achieve positive outcomes.
  • Conduct follow-up reviews and monitor progress against agreed goals.
  • Assess equipment and environmental needs to promote safe discharge and prevent hospital admissions where appropriate.
  • Delegate and provide guidance to Rehabilitation Assistants and Support Workers.
  • Maintain accurate clinical records in line with professional and organisational standards.
  • Participate in multidisciplinary meetings, service development initiatives and continuous learning opportunities.
  • Contribute to a positive team culture focused on delivering high-quality patient care.
The Ideal Candidate

We are looking for an enthusiastic Occupational Therapist who is committed to delivering outstanding patient care and developing their clinical skills within a supportive multidisciplinary environment.

You will:

  • Hold a Degree or Diploma in Occupational Therapy.
  • Be registered with the Health and Care Professions Council (HCPC), or be awaiting registration as a final-year student due to qualify.
  • Hold a full UK driving licence and have access to a vehicle for work purposes.
  • Demonstrate excellent communication and interpersonal skills.
  • Be organised, adaptable and able to manage a varied caseload.
  • Have a patient-centred approach and a genuine commitment to supporting individuals to achieve greater independence.
  • Be confident using IT systems, including Microsoft Office applications such as Outlook, Word and Excel.
  • Be willing to work flexibly across service areas to meet patient and service needs.

Previous experience within rehabilitation, intermediate care, community services or inpatient settings would be beneficial, although newly qualified Occupational Therapists are encouraged to apply and will be fully supported through our Preceptorship Programme.
