What You Will Do: Conduct robust assessments and formulate care plans for individuals aged 16 and up in the Emergency Department and inpatient wards of our bustling 1200-bedded acute hospital.
Plan and coordinate follow-up care from primary care services to Mental Health Act assessments, ensuring continuity and quality of care.
Collaborate with various agencies to gather necessary information, contributing to comprehensive mental health assessments and care planning.
Maintain detailed and accurate records of assessments, treatment plans, and liaison activities to support patient care and service delivery.
As a Registered Mental Health Nurse (RMN), Occupational Therapist (OT), or Social Worker, you will bring experience in assessing a variety of mental health presentations, ideally both functional and organic, in patients 16 years and older.
Your role will drive forward effective mental health interventions and support across our services.
