Job summary

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Main duties of the job

As a member of the Urgent Care team, you will play a vital role in supporting patients to remain safe, well and independent within their own homes and community settings. Working as part of a multidisciplinary team, you will deliver high-quality, person-centred care, ensuring care is provided in line with agreed care plans, risk assessments and professional standards.

You will provide practical and compassionate support, including personal care, meal preparation and medication administration, while carrying out clinical observations and delegated procedures. A key part of the role is recognising changes in a patient's condition, escalating concerns appropriately and providing timely feedback to senior clinicians.

You will also support the triage of referrals, undertake basic and follow-up assessments, develop care plans and risk assessments, arrange equipment and make onward referrals where required. In addition, you will gather patient feedback to support service improvement and maintain a strong understanding of Trust policies and procedures to promote patient safety and quality care.

Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Person specification

Qualifications

Essential

• Evidence of Foundation Degree Level study (Level 5) or equivalent (Nursing Associate) or be able to demonstrate significant experience as a Senior Healthcare worker that is relevant to the job description.
• Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C or equivalent in English and Maths)
• Full UK/EU driving licence

Desirable

• mentorship/ coaching qualification

Experience

Essential

• Evidence of experience as a Senior Healthcare worker
• Experience of working as part of a team
• Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E roster / AT-P / Rio)

Desirable

• Experience of community environment

Knowledge

Essential

• Awareness of professional issues and recent developments in the NHS and Primary Care including Clinical Governance, NICE, Essence of Care, National Urgent Care Agenda
• Knowledge of up to date clinical practice
• Understanding of the role and remit of a Band 4 assessor

Ability to Manage

Essential

• Ability to prioritise care delivery in a professional manner, considering Trust policies and procedures
• Able to support the implementation of local and national agendas for health under the guidance of the Registered Practitioner
