Care Coordinator

to join our friendly and growing domiciliary care team in Barnet. As a Care Coordinator, you will play a vital role in ensuring our clients receive safe, high-quality, person-centred care by coordinating care packages, supporting our care staff, and maintaining excellent communication with clients, families, and healthcare professionals. Key Responsibilities

Coordinate and schedule care visits, ensuring continuity of care and efficient use of staff resources. Develop and maintain effective staff rotas using care management systems. Liaise with clients, families, healthcare professionals, and external agencies to ensure care needs are met. Support with new care package assessments and ongoing reviews. Monitor care delivery and respond promptly to changes in client needs. Provide day-to-day support to the care team, including handling emergencies and last-minute rota changes. Ensure accurate documentation and compliance with CQC standards and company policies. Participate in the on-call rota, where required. About You

Previous experience as a Care Coordinator within a domiciliary care setting is essential. Good understanding of CQC regulations and person-centred care. Excellent organisational, communication, and problem-solving skills. Experience using electronic care planning and rostering systems. Ability to prioritise workload and work effectively in a fast-paced environment. Full UK driving licence and access to a vehicle is desirable. What We Offer
