Job summary
Salisbury Medical Practice is looking to recruit a committed, flexible and motivated Care Home & Frailty Service Lead.
The Care Home & Frailty Service Team is a project working across Salisbury Medical Practice and Harcourt Medical Centre. The team provides a service to the local care homes supporting them in delivering care to patients. We also extend this service to those with learning disabilities and mental health needs (residing in care homes).
This is a permanent position for 25 hours per week working 5 hours a day Monday to Friday.
We offer 5 weeks annual leave (pro rata) and the chance to join or continue on in the NHS Pension Scheme.

Main duties of the job
The Care Home & Frailty Service Lead will be responsible for the general day to day running and management of the Care Home & Frailty Service. The role will involve independent and autonomous working as well as working cohesively as part of a team.
We are looking for someone who has had previous leadership/management experience and has excellent organisational and communications skills as well as a high level of competency using Microsoft Office products and computers. The successful candidate will have the ability to liaise with staff at all levels and work effectively as part of a team. A strong commitment to personal and professional development and teamwork are essential for this role.
About us
We are a friendly, forward thinking and supporting Medical Practice with a large clinical team consisting of GPs, Nurses, Nurse Associates, HCAs, Pharmacists, a Pharmacy Technician, and a First Contact Physiotherapist.

Key Duties
Ensuring the Care Home & Frailty Service team work efficiently and effectively to provide a high quality service to the care homes and their residents working in line with the commissioned service contracts and team KPI's.
To plan, allocate and monitor the workloads for the team ensuring all professionals receive appropriate support and supervision.
To create and update reports on predicted activity against actual activity.
To perform staff appraisals and one-to-ones to mentor and support the team.
To conduct return to work interviews.
To evident that the team are clinically competent and safe to perform the duties of their job role (may require support from Clinical Mentor).
To log and create significant learning events as and when they happen following the correct process.
To help support the team in building and maintaining relationships with outside organisations (e.g. the Care Homes).
To chair and record accurate minutes of meetings, ensuring that these are sent out to the relevant people in a timely manner.
To sign off on annual leave and study requests.
Ensuring the Practice Managers of SMP and HMC are aware of any matters arising or problems / potential problems.

- Demonstrate sufficient knowledge of the English language to communicate effectively with staff and patients
