Job summary

We are looking for a Care Navigator to join our friendly and busy Practice.

You will be the first point of contact for our patients and will provide a crucial role in providing excellent customer care service.

The ideal candidate will be organised, efficient, have excellent verbal and written communication skills, and be able to work well within a team as well as on their own.

We are open for the successful candidate to work either full-time or part-time. Please state in your application how many hours per week you would be able to work.

Main duties of the job

To be the first point of contact for our patients, either face-to-face or by telephone.

Booking appointments for patients.

Processing repeat prescriptions manually and electronically.

Processing emails and electronic letters from Secondary Care.

Sorting and distribution of external post.

Scanning documents.

Filing of patient notes.

Allocation of blood results to GPs.

Informing patients of results from Primary Care and Secondary Care.

Processing results.

Tasks from all clinicians, externally and internally.

To use the AccuRx system for patient communication.

Process patient registrations and change of personal details requests.

Processing consent forms and online access.

To liaise with all members of the Practice Team.

Registering new patients at the Practice.

To set-up and close the Practice, on a rota basis.

Job responsibilities

Job Title: Care Navigator

Responsible to: Support Manager

Job Purpose: To ensure the Reception areas in our Practice run smoothly, provides a high level of service to patients, doctors, nurses and other members of the Practice team.

Duties and Responsibilities

Telephone Duties:

1. Care Navigating patients to the right clinician when booking appointments.

2. Dealing with queries from pharmacies, Secondary Care (e.g. Sheffield Teaching Hospitals) and other external private healthcare providers.

3. Communicating results to patients.

4. Ambulance transport bookings including blue light ambulances.

5. Home visit requests.

6. Dealing with general enquiries, and patient feedback.

7. Contacting patients and other NHS staff.

Reception Duties:

8. Taking new patient registration forms and checking they are accurately completed.

9. Greet and direct patients and visitors.

10. Monitor Reception and Waiting Room areas including making patients aware of clinic updates.

11. Making appointments for patients and directing to them to the right clinician for the appropriate medical need.

12. Opening and dating post and distributing to the right staff team.

13. Ensure outstanding queries are explained and passed to the appropriate staff team, as required.

14. Respond to needs of Clinical Team during opening hours of the Practice.

15. Processing patient requests for online services and subject access forms.

16. Collect payments from patients for non-NHS services.

17. Accepting samples from patients and making sure adequately labelled.

Prescriptions:

18. Processing patient repeat and one-off prescriptions.

19. Resolving prescription queries.

General:

20. Be responsible for the opening and closing of the building in the morning and evening, including arming and disarming the alarm system.

21. Input and extract information from the Practice's software systems.

22. Always observe health and safety guidelines.

23. General housekeeping in staff and patient areas.

24. Admin duties to include processing tasks, AccuRx system, allocation of blood tests to GP's and processing the acute and medication review list.

25. Task allocation by Support Manager or one of our Reception Supervisors.

Person specification

Qualifications

Essential

  • GCSE grades A to C, preferably in English and Maths

References

Essential

  • Two proven references are required before a confirmation of start date is given, as per NHS employment guidance.

Experience

Essential

  • Previous SystmOne experience.
  • Excellent communication skills, both verbal and written.
  • Experience working in a customer service role.
  • Experience using Microsoft Office skills, Outlook, Word and Excel.
  • Experience of working in a multi-disciplinary team.

Desirable

  • Previous NHS experience.
  • Experience of working with confidential material.
