Social Worker – Leaving Care Team Wrexham | £38.43 per hour (Umbrella) | 6-Month Contract Wrexham Children’s Services are looking for an experienced Social Worker to join their established and high-performing Leaving Care Team, supporting care-experienced young people aged 15–25 as they transition towards independence. This is an excellent opportunity for a committed practitioner who is passionate about relationship-based, trauma-informed practice and helping young people achieve positive outcomes. Key Responsibilities * Support care-experienced young people with transition and pathway planning. * Complete LAC reviews and statutory visits. * Promote independence, resilience and positive outcomes. * Apply corporate parenting principles and trauma-informed approaches. * Build strong, trusting relationships with young people. * Work collaboratively with families, professionals and partner agencies. * Model best practice and provide mentoring and support to colleagues. * Maintain accurate case records and ensure statutory requirements are met. * Apply relevant Welsh legislation, guidance and social care frameworks. Essential Requirements * Recognised Social Work qualification – essential. * Experience working with children, young people or care-experienced young people. * Experience completing LAC reviews and statutory visits. * Good understanding of Leaving Care and transition services. * Knowledge of relevant Welsh legislation and children’s social care requirements. * Strong relationship-based and trauma-informed practice skills. * Ability to work effectively within a supportive, high-performing team.
