Role Details * £34.00 per hour * Full-time locum * ASAP start * Flexible working pattern * Hybrid working available * Combination of office, home and community-based working About the Role You’ll support young people in care and care leavers as they develop independence skills and prepare for adulthood. Key responsibilities include: * Completing assessments and personalised pathway plans * Supporting access to education, employment, accommodation and health services * Promoting the voice and aspirations of young people * Working with housing, education, health and community partners * Undertaking visits, reviews and statutory visits * Assessing risk and identifying support needs * Advocating for young people's rights and wellbeing * Maintaining accurate case records and documentation Why SW Locums?
