Job summary

An exciting opportunity has opened for an ambitious, dynamic and enthusiastic Clinical Exercise Physiologist to join the cardiovascular rehabilitation team at UHCW NHS Trust. We are seeking a highly motivated candidate to join our dynamic and high performing multi-disciplinary cardiovascular rehabilitation service.

This multi-site service offers cardiovascular rehabilitation to the following groups of patients - STEMI, NSTEMI, Revasc, HFrEF, HFpEF and PAD across the patient recovery pathway.

This post is for initially 12 months.

Hours 22.5 hours a week over 3 days (0800-1600)

Secondment will be considered for internal candidates - If you are successful after interview, you must obtain approval from your current line manager to accept the offer.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job

  1. A skilled and knowledgeable practitioner in exercise science applied to a range of long-term conditions and morbidities including cardiovascular, metabolic, respiratory and cancer.
  2. Accountable for own actions in accordance with the REPS and NHS Code of conduct.
  3. Assist in the management and day to day running of the community and hospital multi-morbidity rehabilitation programme.
  4. Participate in the development of quality initiatives, including standards of the multi-morbidity rehabilitation programme, patient care, clinical audit, monitoring of service provision and clinical research.
  5. Maintain and develop clinical expertise by attendance and participation in regular post-registration education as identified by PDP's i.e. BACPR/BASES study days or conferences and in-service training, participating in mentorship and appraisal as required.

Job responsibilities

  • Ability to undertake a clinical assessment (as an autonomous specialist) of patients with long-term conditions, including those with complex presentation, investigate, analyse & formulate individualised management & treatment plans to formulate specialised programmes of care.
  • To identify and respond appropriately to additional long-term conditions and/or co-morbidities and the potential effect these may have on physical activity and lifestyle advice. Use this information appropriately prior to advising, planning or implementing physical activity and lifestyle treatment plans.
  • Accept clinical responsibility for own caseload & organise this effectively & efficiently with regard to the clinical progress of the patient.
  • Maintain immediate life support and defibrillation training ILS.
  • Collaborate with long term condition specialists and all members of the multidisciplinary team in the primary and secondary health care settings to ensure patients are receiving the highest quality of care.
  • Ensure patient information is collated, monitored and fully accurate maintaining complete records in line with Trust standards.
  • Promote the involvement and engagement of patients, members and carers/relatives in the assessment, delivery and evaluation of care.

Person specification

Qualifications

Essential

  • oBSc sports science or equivalent e.g. BSc Physiotherapy oMSc in cardiac rehabilitation, clinical exercise physiology, physiotherapy or related field

Desirable

  • oOther level 4 exercise qualification in long term condition, i.e. British lung foundation, Cancer oBACPR phase lV exercise instructors award oGym instruction/personal training Level 3 oAHCS registration in Clinical Exercise Physiology (or working towards registration)

Experience

Essential

  • oClinical experience working with cardiac patients or patients with other long-term conditions.

Desirable

  • oExperience of dealing in partnership with external stakeholders (volunteers, support groups)

Knowledge

Essential

  • oPathophysiology, aetiology and epidemiology of coronary/vascular/ respiratory disease. oUp to date of latest evidence and current best practice in cardiac/vascular/ heart failure rehabilitation

Desirable

  • oOrganisational infrastructure of the NHS and the health care system. oHealth and Safety oNHS patient information systems. oMedico legal implications of the provision of exercise programmes for individuals with long-term conditions.

Skills

Essential

  • oSubmaximal fitness assessment and risk stratification of a clinical population oExercise prescription and review oSafe and effective fitness instruction to patients with long term conditions.

Desirable

  • oDeliver patient education sessions. oManagement of physical resources.
