Clinical Lead - Adult Community

We are searching for an Adult Clinical Lead Nurse to join an award-winning community complex care provider based in Stoke-on-Trent. As an Adult Clinical Lead you will: Create and manage tailored care plans for adult clients with complex care needs Work in partnership with CHC nurses, commissioners, and wider MDT to maintain bespoke care packages Registered Nurse (RGN) with an active NMC PIN Full UK driving licence Good understanding of CQC standards & clinical governance
