Community Nurse
Job summary
Working with your community nursing team, you'll help deliver safe and person-centred care that supports people to stay well at home.You'll be part of an integrated service that provides high-quality nursing care to adults with a wide range of needs. Your work will help people manage their health, stay independent and feel supported in their own homes and residential settings. You will join a friendly and committed team who work closely with colleagues across the Integrated Nursing and Conditions services. We provide a supportive environment where you can grow your clinical skills and contribute to continuous improvement. What we're looking for:- A registered nurse with current NMC registration
- Good assessment, communication and clinical skills
- Ability to work well in a team and build positive relationships
- An interest in community nursing; you may be newly qualified or experienced
- A willingness to learn, develop and adapt to changing needs
Main duties of the job
What you'll do:- Carry out holistic assessments and develop clear, person-centred care plans
- Deliver safe and effective nursing care in line with each person's needs
- Work with colleagues and partners to support joined-up community care
- Use and update the Electronic Patient Record (EPR) to maintain accurate notes
- Support people, families and carers to understand and manage health needs

- Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration

- Demonstrate an understanding of the role of a community nurse
- Knowledge of legislation impacting on service delivery
- Knowledge of the modernisation agenda
- Demonstrate an understanding of evidence based practice
- Knowledge of local initiatives
- Demonstrate an understanding of the MDT
- Basic IT skills
- Knowledge and skills in the assessment process
