Job summary

An exciting opportunity has arisen for an innovative role of Mental Health Practitioner with the Older Adult Mental Health team Brecon, Powys.

This role would also provide an opportunity to be mentored or to mentor, career progression and continued education.

We are seeking applications from Registered Mental Health Nurses, Social workers or Occupational Therapists, who have experience of working in community services.

Main duties of the job

Responsible for the specialist assessment and care of clients and provide relevant appropriate treatments to allow them tomaintain independence for as long as possible in the community. Liaise with specialist services within their catchment area and develop links with Primary Care Services and other caregivers.Manage outcomes autonomously; link to peer support through supervision and video conferencing facilities.

Qualifications and Knowledge

Essential

  • Registered Mental Health Nurse OR Occupational Therapist OR Registered Social Worker
  • Up-to-date registration
  • Post registration experience within Older Age Psychiatry Assessment processes
  • Skilled in risk assessment and risk management
  • Understanding of the Mental Health Measure (Wales)
  • Knowledge of evidence-based interventions

Experience

Essential

  • Prioritise and manage own workload
  • Multi-disciplinary teamwork

Skills and Attributes

Essential

  • Knowledgeable and skilled in mental health assessment, requiring the ability to plan, organise and signpost care
  • Proven clinical skills commensurate with the post
  • Skilled in care and treatment planning

Other

Essential

  • Resilient in dealing with patients' experiences of extreme distress, and supporting families
  • Ability to travel within geographical area
