Job summary

The service triages and assesses all urgent and emergency referrals to identify the most appropriate service to meet individual need when persons are experiencing a mental health crisis. This involves telephone triage, support and signposting and face to face full needs assessment care planning and referring on to relevant services.

The team currently consists of Community Mental Health Nurses, Psychologist,Consultant Psychiatrist and other associated medical staff.

The team provide a service over a 24/7 period encompassing a varied shift pattern.

There is a possibility of cover into the Home treatment team, who provide intensive support at home, and an early discharge pathway to facilitate discharge from the wards.

Main duties of the job

To provide a clinical service that is service user focussed.

To triage all mental health referrals to the Crisis Team to determine the priority for service response and the appropriate clinical pathway.

To carry out full assessments of needs including comprehensive risk assessments of persons referred or self -referred to the service.

To recommend and refer, where treatment/intervention is indicated, to the most appropriate team/ service utilising all available resources.

To identify where appropriate treatment based interventions for all service users ensuring that best practice is implemented.

Person specification

Qualifications

Essential

  • Mental Nursing Certificate (RMN) Registration on NMC
  • Evidence of post registration training

Experience

Essential

  • Demonstrable post registration experience as an RMN
  • Professional experience of working in acute adult mental health.
  • Experience of MDT working
  • Skills in coordinating and planning inter-agency approaches in mental health care
  • Experience of assessing and managing risk

Desirable

  • ENB 998 or other NMC approved mentor qualification or mentor qualification appropriate to discipline- or willingness to complete.

Skills

Essential

  • Excellent report-writing skills
  • IT skills.
  • Ability to work as part of a multi-professional team
  • Understanding of the CPA process
  • Excellent assessment skills, including risk assessment
  • Understanding of the importance of user/carer involvement in care planning
  • Experience of Safeguarding

Knowledge

Essential

  • Ability to use initiative and work without immediate supervision
  • Ability to demonstrate an understanding, empathic approach
  • Skills in co-ordinating and planning interagency approaches in mental health care
  • An understanding of current mental health issues
