Main duties of the job

We are a compassionate and person-centred Adult Community Team supporting people aged 18 and over who are living with physical disabilities, chronic and long-term conditions, autism or sensory impairments.

You will join an experienced and supportive team where teamwork, wellbeing and mutual respect guide how we work. In this varied role, you will work closely with individuals, families and carers, always taking a person-centred, empathetic and adaptable approach. You will complete Care Act 2014 assessments, identify needs and manage both short-term and long-term cases. Your work will also include progressing safeguarding enquiries, supporting Court of Protection matters and working closely with health and multidisciplinary partners. Agility and professionalism are essential as you respond to changing needs with compassion and integrity.

You must be registered with Social Work England, understand the Care Act 2014 and take a positive, balanced approach to risk.
