HeadofAgeWellThispostisjointfundedbytheNHS,itisessentialthatcandidateshaveexperienceofworkingintheNHSoranintegratedcommissioningfunction.ItisessentialthatcandidateshaveexperienceofmanagingNHSContinuingHealthCare(CHC)teamandBetterCareFund(BCF)initiativesincludingdevelopingproposalsforBCFandreportingtheNHSEngland.

HeadofAgeWellJobdescription,keypurpose
HeadofAgeWellThisroleisakeypartofthecommissioningteamwhichisfundedbySouthwarkCouncilandSELICS.Theroleleadsacommissioningteamfocusedonolderand/orfrailresidents,includingphysicaldisabilities,ofSouthwark.Thepostholderwillhaveasignificantroleinbuildingworkingrelationshipsandcollaborationacrossthecouncil,thewiderhealthsystemandthroughPartnershipSouthwark.
AspartofthisteamthepostholderwillleadandthereforesignificantlycontributetodevelopinganddeliveringaspecifiedprogrammeofworkwhichwillfocusonadefinedpopulationwithinSouthwark.Thepostholderwillbeexpectedtocontributetotheprofessionaldevelopmentoftheteamandco-ordinatethedeliveryofacomplexprogrammeofworkcentredoncorehealthandcareprioritiesinSouthwarkandSouthEastLondon.
HeadofAgeWellwillalsosignificantlycontributetoplanningforcontinuousreductioninhealthandwellbeinginequalitiesacrosstheboroughincollaborationacrossthecouncil,thewiderhealthsystemandthroughPartnershipSouthwark.Workingacrossthedivision,thepostholderwilltakestockofthecurrentcommissioningarrangementsandthestatusofpublichealth-commissionedservices,engagingwithPublicHealthConsultants,HeadsofProgramme,andPolicyOfficerstodevelopandimplementacommissioningplanforthedivision.
HeadofAgeWellshouldassess,rationalise,anddeliverallaspectsofhighqualitypublichealthcommissioningwithaviewtotacklinginequalities,improvingoutcomesandgeneratingvalueforSouthwarkresidents.Thepostholderwillberesponsibleforcreatingandsustaininggoodworkingrelationshipsinsupportofcommissioningkeypublichealthservicesandtheirlinkswithotherrelatedservices,withintheCouncil,theNHS,voluntarysectorandotherrelevantlocalpartners,aswellascommissionedserviceproviders.Thepostholderwillberequiredtoattendoccasionaleveningandweekendmeetingsandeventsinavarietyofsettingsandlocationsacrosstheborough,insupportofthedeliveryoftheirworkprogramme.ThepostholderreportstotheAssistantDirectorforIntegratedCommissioning.
