HeadofAgeWellThispostisjointfundedbytheNHS,itisessentialthatcandidateshaveexperienceofworkingintheNHSoranintegratedcommissioningfunction.ItisessentialthatcandidateshaveexperienceofmanagingNHSContinuingHealthCare(CHC)teamandBetterCareFund(BCF)initiativesincludingdevelopingproposalsforBCFandreportingtheNHSEngland.

HeadofAgeWellJobdescription,keypurpose

HeadofAgeWellThisroleisakeypartofthecommissioningteamwhichisfundedbySouthwarkCouncilandSELICS.Theroleleadsacommissioningteamfocusedonolderand/orfrailresidents,includingphysicaldisabilities,ofSouthwark.Thepostholderwillhaveasignificantroleinbuildingworkingrelationshipsandcollaborationacrossthecouncil,thewiderhealthsystemandthroughPartnershipSouthwark.

AspartofthisteamthepostholderwillleadandthereforesignificantlycontributetodevelopinganddeliveringaspecifiedprogrammeofworkwhichwillfocusonadefinedpopulationwithinSouthwark.Thepostholderwillbeexpectedtocontributetotheprofessionaldevelopmentoftheteamandco-ordinatethedeliveryofacomplexprogrammeofworkcentredoncorehealthandcareprioritiesinSouthwarkandSouthEastLondon.

HeadofAgeWellwillalsosignificantlycontributetoplanningforcontinuousreductioninhealthandwellbeinginequalitiesacrosstheboroughincollaborationacrossthecouncil,thewiderhealthsystemandthroughPartnershipSouthwark.Workingacrossthedivision,t...
