Salary: From £28,000 (Dependent on Experience)

Hours: 37.5 hours Monday to Friday (part time considered)

Base: Penn Surgery HP10 8LQ with travel across Arc Bucks PCN practices

Main duties of the job

We're looking for an enthusiastic and compassionate Health & Wellbeing Coach to join our multidisciplinary team at Arc Bucks Primary Care Network. You'll work alongside GPs, nurses, pharmacists, social prescribing link workers and care coordinators to support patients to take control of their health and achieve goals that matter to them.

Whether someone wants to lose weight, reduce their risk of diabetes, improve their mental wellbeing, become more active or better manage a long-term condition, you'll provide the coaching, encouragement and practical support to help them succeed.

You'll have the opportunity to build meaningful relationships with patients, deliver innovative health programmes and contribute to improving the health of our local communities.

You'll be trusted to coach rather than instruct.

You'll be encouraged to innovate rather than simply follow a script.

Most importantly, you'll see first-hand the difference your work makes to people's lives.

Purpose of the Role

To empower people to improve their health and wellbeing through personalised coaching, behaviour change and self-management support.

Working as part of the multidisciplinary team, you'll help patients develop the confidence, knowledge and skills to make sustainable lifestyle changes, improve long-term health outcomes and reduce health inequalities.

FedBucks is a not-for-profit GP federation of 45 GP practices covering a population of over 550,000 patients across Buckinghamshire. We began in 2016 and now employ around 300 members of staff at our head office site, and across our planned and unplanned care services.

Job responsibilities

Key Responsibilities

Personalised Health Coaching

  • Provide one-to-one health coaching using personalised care and behaviour change techniques.
  • Support patients to identify what matters most to them and develop achievable health goals.
  • Deliver ongoing coaching sessions that encourage confidence, resilience and long-term self-management.
  • Empower patients to become active participants in their own healthcare.

Long-Term Condition Prevention and Management

  • Support patients living with or at risk of long term conditions including diabetes, hypertension, cardiovascular disease and obesity.
  • Deliver evidence-based lifestyle interventions to improve physical and mental wellbeing.
  • Work alongside clinical teams to promote prevention, early intervention and healthier lifestyles.
  • Support delivery of weight management and healthy lifestyle programmes.

Community and Partnership Working

  • Work closely with Social Prescribing Link Workers and the wider multidisciplinary team.
  • Connect patients with local community activities and wellbeing services where appropriate.
  • Help establish peer support groups and community-based wellbeing initiatives.
  • Build strong relationships with voluntary organisations and community partners.

Health Promotion

  • Deliver health education sessions, group programmes and community events.
  • Promote healthy lifestyles across a range of patient groups.
  • Encourage shared decision making and personalised care planning.
  • Support patients to improve confidence in managing their own health.

Quality Improvement

  • Maintain accurate clinical records using GP clinical systems.
  • Collect outcome data to demonstrate the impact of coaching interventions.
  • Contribute to QOF, IIF and wider quality improvement programmes.
  • Participate in service development and continuous improvement initiatives.
