Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

As a Healthcare Assistant in a community NHS trust, you'll play a vital role in delivering personalised care within integrated neighbourhood teams, working alongside nurses, GPs, therapists, social workers, and voluntary services. As a Care Coordinator, you'll be a key point of contact for patients, carers, and professionals, ensuring joined-up support across health, social care, and voluntary services. You'll manage a caseload, coordinate care, and help patients access the right services to maintain their independence and wellbeing.
In this rewarding role, you will:
- Support housebound patients living with long-term conditions.
- Help reduce avoidable hospital attendances and admissions.
- Respond to referrals and coordinate access to services.
- Provide advice and support to patients, carers, and colleagues.
- Develop personalised plans that promote independence and prevent care crises.
- Empower patients to proactively manage their health and wellbeing.

Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Qualifications, training and experience
Essential
- Level 4 qualification (e.g. certificate of higher education, higher national certificate, NVQ level 4) Or you will be able to demonstrate the equivalent level of experience
- You'll need to have recent experience of team working and versatility to take on a variety of tasks
- A full, valid UK driving licence or a full driving licence issued by an EU country
Knowledge and skills
Essential
- You will need to have a customer focussed approach which is essential to; Provide patients, clients and their families with the best possible experience
- You will need to be able to be able to use IT systems and databases.
- You'll need have sound communication skills in order to understand and communicate both verbally and in writing. Which at times may also require gaining cooperation from others through tact and persuasive skills particularly where there are barriers to understanding
Desirable
- Having an awareness of developments within the NHS would be an advantage
