Rainbow Care Solutions is a long-established, CQC-registered home care provider with over 20 years experience. Were recruiting Full-Time Home Care Assistants (37.5 hours/week) to support our Discharge to Assess (D2A) / Reablement service across Herefordshire.
This role supports people returning home from hospital to recover safely and regain independence
doing with, not doing for. Location
Herefordshire (community-based) What youll be doing
Supporting people at home after discharge with reablement-focused care
Assisting with personal care where needed, while encouraging independence
Supporting daily living skills (washing, dressing, meal prep, mobility, routines)
Following care plans and recording notes accurately
Reporting changes in condition, concerns, or risks promptly
Working respectfully with families and professionals as part of a wider team What were looking for
Caring, reliable and professional approach
Experience in home care is helpful, but not essential (full training provided)
Good communication and respect for dignity, choice and independence
Full UK driving licence and access to a vehicle (essential for Herefordshire)
