Working within a Housing First, trauma-informed and harm reduction model, you will provide person-centred support to individuals experiencing homelessness and complex needs.
You will help service users sustain accommodation, improve wellbeing and access services that promote long-term independence.
Key Responsibilities Deliver one-to-one support and key working sessions.
Complete needs and risk assessments.
Develop and review support plans.
Support tenancy sustainment and resettlement.
Advocate for service users and coordinate support services.
Maintain accurate records and reports.
Undertake outreach and community-based support work. xwzovoh
Essential:
* Level 3 Social Care qualification and 2 years' experience with vulnerable adults OR
* 3 years' experience with vulnerable adults and Level 2 English & Maths
* Knowledge of homelessness and support planning
* Good communication and IT skills
* Full driving licence and access to a vehicle
