Job summary

The Integrated Wellbeing and Bereavement Service Coordinator will play a key role in delivering compassionate, person-centered support to patients, families and carers from diagnosis, throughout illness and into bereavement. Working as part of our Integrated Wellbeing and Bereavement Team, you'll coordinate holistic support that helps people live well, navigate loss and access the right care at the right time.

Main duties of the job

Alongside managing an allocated caseload, you'll work closely with colleagues, volunteers and partner organisations to strengthen community connections and help build compassionate communities across Furness and South Lakes. You'll support the development of innovative wellbeing and bereavement services, helping people access the emotional, practical and social support they need, while working collaboratively with local organisations to reduce isolation and improve wellbeing. 30 hours per week
