About the Role As a My Way Facilitator (Transitions Co-ordinator) , you will walk alongside young people and their families as they move into, through, and out of our No Limits provision – our bespoke further education programme.
You will build strong relationships with young people, families and professionals, ensuring every learner receives consistent, person-centred support that reflects their strengths, aspirations and individual needs.
As part of the No Limits team, you will work closely with colleagues, families and external partners to ensure young people experience positive transitions into, through and beyond our provision.
Coordinate person-centred transition planning for young people entering, progressing through and leaving No Limits.
Work collaboratively with local authorities, education providers, social care teams and health professionals.
Attend and contribute to EHCP reviews and multi-disciplinary meetings.
Help ensure every learner leaves No Limits with a clear and ambitious next step that reflects what matters most to them.
Promote independence, self-advocacy, community inclusion and citizenship throughout the learner journey.

Additional Requirements Full UK driving licence.
Willingness to travel across Nuneaton and surrounding areas as required.

#Experience in education, health, or social care, particularly supporting children and young people with learning disabilities and/or autism.
Excellent project planning and IT skills , and the ability to coordinate effectively with a wide range of professionals.

Training and Development
