Job summary

The Lead Nurse will provide visible clinical and professional leadership of the integrated discharge team, the Lead Nurse post is a senior leadership role focused on patient flow, complex discharges, and delayed discharge reduction.The post holder will be responsible for leading and managing the Hillingdon Hospital Integrated Discharge Team, delivering a robust integrated discharge model that ensures effective, person-centred and integrated pathways of care out of hospital are embedded through interdisciplinary ways of working and clear, well managed processes and protocols.

The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust's services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.

Essential

  • Currently registered with the NMC
  • Master's Level or working towards
  • Experience of working at a senior level and working collaboratively with external stakeholders e.g. partners in primary care, local authorities, other NHS organisations
  • Experience of leading significant project work and managing service changes.
  • Experience of service development and quality improvement
