Job summary

As the Living Well After Stroke (LWAS) Support Worker for Mendip, you will be expected to provide a holistic review of stroke needs six months post stroke. You will manage a large caseload of people recovering from stroke in the Mendip area. You will be responsible for addressing their holistic post stroke needs, queries and concerns along their stroke recovery journey. You will be part of the LWAS Team consisting of a Team Lead, LWAS Nurse and two Support Workers covering the South and West of the county. Our LWAS team forms part of the wider Integrated Community Stroke Service in Somerset. The role is 26 hours a week across four days. There is no weekend or shift work.

Main duties of the job

The main duty of this role is to provide a holistic review of stroke needs six months post stroke. You will manage a large caseload of people recovering from stroke in the Mendip area. You will be responsible for addressing their holistic post stroke needs, queries and concerns along their stroke recovery journey. Experience of working in a care setting would be advantageous but all necessary skills training, professional updates and mandatory training as appropriate to the post will be provided. Person specification

Qualifications

Essential

  • Good standard of literacy and numeracy and spoken English (Minimum Level 2, or Functional Skills).
  • Vocational Qualification Level 3 equivalent level or equivalent experience.
  • Level 3 qualification or Certificate in Healthcare,
  • Care Certificate or willing to work within agreed time scale.
  • Evidence of working independently
  • Ability to use IT systems
