Job summary
We are seeking a qualified Practice Nurse, ideally with experience in a GP setting, to join our dedicated and supportive team.
As a valued member of the practice, you will work collaboratively alongside GPs, Nurses, Paramedics, and other healthcare professionals to deliver high-quality, patient-centred care.

Main duties of the job
The post holder will play a key role in the management of patients with long term conditions across the practice population, delivering proactive, patient-centred care and supporting QOF and IIF outcomes.

Provision of adult immunisations.
Promoting health education and self-care
Management of Long Term Conditions - (eg. Asthma, COPD, Hypertension, Mental Health Review, Heart Failure) or willingness to train
Contributing to QOF, enhanced services, and practice development initiatives
Work within the SystmOne Clinical system to record consultations, manage recalls, and contribute to clinical audits.
Collaborate with GPs, pharmacists, practice nurses, HCAs and external services.
Support the practice in meeting QOF, IIF, and other long-term condition targets.
Provide ongoing education and lifestyle advice to empower patients in self-management.
Support service development and audit quality improvement initiatives.
Person specification
Qualifications
Essential
- Registered Nurse with a valid NMC registration. Experience in delivering vaccinations and experience managing patients with long term conditions within primary or community care. Excellent communication, time management, and patient education skills, with a focus on empowering patients. Able to work independently while contributing effectively within a multidisciplinary team. Compassionate, proactive, and patient-centred approach to care.
Desirable
- Desirable criteria
- Knowledge and experience of SystmOne. Confident in managing some or all chronic disease areas.
- Preferably with Diploma or Degree in Long Term conditions Management.
