We are looking for a Mental Health and Wellbeing Coach to join our team at YOURhealth in Bridlington

The role of the Mental Health and Wellbeing Coach is to work as part of Humber Teaching NHS Foundation Trusts YOURhealth service in the Bridlington locality with the aim of improving the mental health and wellbeing the local community.

The role will support the Bridlington Primary Care Networks and patients within to access low to moderate mental health support through 1-1 support to help them move from surviving to thriving. The aim being to help people feel in control of their health and wellbeing by addressing mental health, general health, wellbeing and inclusion needs using a behaviour change approach and active recovery methodology including the Recovery Star with continuous goal setting and action planning throughout their time with the service.

The role of the Mental Health and Wellbeing Coach is a role integrated as part of the YOURhealth team whilst working clinically with the Primary Care Mental Health Network in Bridlington. You will be required to work mostly at our YOURhealth hub site on Quay Road.

Main duties of the job

  • To provide individual support for people within primary care to improve mental health and wellbeing
  • Support people to utilise self-help and facilitated tools such as the recovery star to aid individual action planning and goal setting with measurements of recovery throughout.
  • Support people to make informed choices about their health and wellbeing
  • Support access to wider community activities
  • Support primary care and mental health colleges to address the wider determinates of health
  • Undertake multi agency working
  • Develop and maintain effective relationships with local communities primary care , secondary mental health service, VCSE sector, recovery college, Social Prescribers etc.
  • Ensure performance information is collected accurately and timely
  • To actively support the Primary Care Mental Health Network in fulfilling all contractual requirements
  • Safely and effectively manage own caseload under supervision of Mental health network leads

Person specification

Knowledge and Experience

Essential

  • Demonstrable experience of working within the identified service area (mental health/physical health)
  • Understanding of health promotion and behaviour change
  • Managing a caseload
  • Minimum 1 year facilitation/coaching experience in mental health
  • Knowledge/experience of prevention and mental health

Qualifications, Education and Training

Essential

  • Successful completion of foundation degree equivalent to diploma level OR NVQ 3 plus significant experience of working with patients with physical and mental needs demonstrating sound use of techniques and good practice
  • A good understanding of physical to Foundation Degree equivalent to diploma level including health promotion, and mental health including policy/legislation
  • IT skills/Qualification

Desirable

  • Full understanding/application of relevant clinical practice/standards/audit within identified clinical area
  • A broad understanding of relevant policy, legislation drivers and their application to clinical and service area

Skills, Competencies and Personal Qualities

Essential

  • Evidence of continued professional development documented through Trust PDR/KSF/Supervision processes and personal portfolio

Desirable

  • Supervisory management skills
