As a Nurse Case Manager, you'll take ownership of a varied caseload, working closely with clients, families and multidisciplinary teams to ensure every individual receives exceptional, person-centred care. The Role You'll oversee complex care packages from initial assessment through to ongoing clinical management, ensuring safe, high-quality care is consistently delivered within the community.
