Job summary

Reedyford Health Care is a high-achieving, busy general practice with a patient list around 14,500, serving our local community. We are seeking an experienced and motivated Nurse Practitioner to join our multidisciplinary team, providing same-day and urgent care for our adults and children presenting with acute and undifferentiated conditions.

Main duties of the job

  • Autonomous assessment, diagnosis, and management of acute presentations
  • Prescribing and ordering/interpreting investigations within scope of practice
  • Management of minor illness, minor injury, and acute exacerbations of long-term conditions
  • Appropriate referral and escalation when required
  • Collaborative working with GPs, nurses, and the wider practice team

Job responsibilities

Key Responsibilities

Clinical Care

  • Independently assess, diagnose, and manage adults and children presenting with acute and undifferentiated conditions.
  • Provide same-day and urgent appointments for acute illness and minor injuries.
  • Prescribe medications and request/interpret investigations in accordance with scope of practice and regulatory requirements.
  • Initiate treatment plans, provide safety-netting advice, and arrange appropriate follow-up.
  • Identify patients requiring escalation of care and arrange referral to secondary or emergency services when necessary.
  • Manage exacerbations of chronic disease presenting acutely (e.g. asthma, COPD, diabetes).

Patient Education & Health Promotion

  • Provide clear advice and education to patients and carers regarding diagnosis, treatment, and self-care.
  • Promote preventive care and early intervention where appropriate.

Multidisciplinary Working

  • Work collaboratively with GPs, nurses, allied health professionals, and administrative staff.
  • Participate in clinical handovers and case discussions.
  • Support and mentor nursing staff where appropriate.

Governance, Quality & Safety

  • Maintain accurate, timely, and comprehensive clinical documentation in the electronic medical record.
  • Practice in accordance with national clinical guidelines, professional standards, and practice policies.
  • Participate in clinical audit, quality improvement, and risk management activities.
  • Maintain professional registration, revalidation, and continuing professional development (CPD).

  • Demonstrated experience managing acute presentations in primary care, urgent care, or emergency settings
  • Strong clinical assessment and diagnostic skills
  • Excellent communication, decision making, and organisational skills
  • Ability to work autonomously while recognising professional limits

Desirable

  • Experience working in general practice.
  • Minor illness and minor injury qualification.

Qualifications

Essential

  • Registered Nurse with current registration with the relevant nursing regulatory body.
  • Qualified Nurse Practitioner Masters level or equivalent with appropriate endorsement.
  • Independent prescribing authority where applicable
