Patient Co-ordinator
Job summary
We are seeking to appoint a Patient Co-ordinators to work 20 hours a week between 8:00AM - 6:30PM with our friendly patient focused team at Ystradgynlais Group Practice.
We are looking for an enthusiastic, reliable, highly motivated and friendly individuals to join our team. The successful candidates would ideally have experience working in General Practice.
Main duties of the job
The successful candidate will be a key member of the patient co-ordinator teams. You will play a vital role in dealing with the patients, both face to face and over the telephone. You will provide support the GPs and clinical team by signposting patients to the appropriate healthcare professional or service. The patient coordinator will also be expected to undertake reception duties and support the administrative team. You will be required to work across our 3 sites.
About us
We operate across 3 sites Pengorof Surgery, Ystalyfera Surgery and Abercrave Surgery. Our team of 9 Partners, 3 Registrars are supported by 3 Advanced Care Planning Nurses, 3 Practice Nurse, 4 Health Care Assistants, 2 Practice Pharmacists, 3 Phlebotomists, 1 Cluster Pharmacist and 2 Cluster Pharmacy Technicians and 4 Pharmacy dispensers. Practice Manager, Deputy Managers and a large team of administrative/clerical staff.
Our practice aims to improve the health of those living in and around the Ystradgynlais area and we work closely with many local community services to achieve this, including the community hospital where our GPs provide cover to the Minor Injuries Unit and work alongside the ward Consultants.
Job responsibilities
To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practices multidisciplinary team. The care navigator will also be expected to undertake reception duties as part of the role, supporting the administrative team in delivering a polite and professional service to the entitled patient population.

Desirable
- Broad knowledge of General Practice Experience working with the general public/patients. Competent in the use of Office and Outlook EMIS user skills Ability to effectively manage stock, including ordering, rotation, disposal etc.
