Job summary

Cobbs Garden Surgery has an exciting opportunity for an experienced practice nurse to join our friendly, forwardthinking team to specialise in chronic disease management, asthma, COPD and cardiovascular disease, so experience in these areas is essential.

This is an excellent role for a skilled nurse who enjoys a broad clinical remit, continuity of care and being part of a supportive multidisciplinary team. The position is offered on a parttime basis for 3 to 4 days per week.

Main duties of the job

  • Carrying out FeNO or Spirometry assessments (training can be provided)
  • Carrying out respiratory and cardiovascular assessments
  • Managing and treating long term conditions in line with national and local guidelines and practice protocols
  • Promoting general health and wellbeing, and creating person centred care plans for patients with, or at risk of, long term conditions
  • Supporting our treatment room team with other duties including; travel injections, immunisations, Cervical screening, HRT and contraception reviews and doppler assessments
  • Providing and evaluating a range of health interventions to meet patients health and social care needs
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence based practice and agreed protocols, and supporting patients with adherence
  • Prioritising health problems and intervening appropriately in complex, urgent or emergency situations, including initiating effective emergency care
  • New patient checks and NHS health checks

You will work closely with our GPs, ANPs, pharmacist and wider team to deliver safe, effective and compassionate care.

Qualifications

Essential

  • A relevant nursing degree.
  • Current NMC registration with an active PIN.

Experience

Essential

  • Competence to undertake asthma, COPD and diabetes clinics and reviews, with relevant certificates
  • Treatment room consultations
  • Screening and management of long term conditions
  • Cervical cytology sampling (desired)
  • Vaccinations and immunisations (desired)
  • Feno and Spirometry assessments desirable but training can be arranged
  • Cardiovascular Assessments (desired)
  • Assisting with minor surgery
  • Dressings and wound care
  • Recording spirometry
