Job Summary

We are looking for a Practice Nurse who will work as part of the Nursing Team, delivering high-quality, patient-focused nursing care. The role involves managing chronic diseases, delivering preventative care, supporting health promotion, and assisting in clinical procedures, in line with NHS standards and guidelines.

Main duties of the job

  • We are looking for an experienced Practice Nurse who is currently confident in
  • Chronic disease management
  • Cervical smear taking
  • Immunisations administration for both adult and children
  • Travel vaccine administration and advice

Earnswood Medical Centre is a well established practice based in Crewe. We are a friendly and forward thinking practice with some of our GP's having specialist interests in Contraception, Education and Minor surgery.

We have a list size of approx. 17,200 patients.

We are a very friendly practice with a wide range of clinical and non-clinical staff. We have 6 partners, 5 salaried GP's, 4 ACP's a Clinical Pharmacy team, Practice Nurse Team, MSK practitioners and Social Prescriber all supported by an experienced management team.

Job responsibilities

Key Responsibilities

Clinical Duties

  • Undertake chronic disease management (e.g. diabetes, asthma, COPD, hypertension) in line with national and local protocols.
  • Administer vaccines and injections safely and effectively.
  • Provide general nursing services including immunisations, and cervical screening.
  • Perform health assessments, screenings, and NHS health checks.
  • Support minor surgery procedures and assist GPs when required.
  • Carry out travel health consultations and vaccinations where appropriate.

Person specification

Qualifications

Essential

  • Experience in a GP practice setting
  • Registered Nurse (RN) with current NMC registration
  • Practice Nurse qualification, Cytology , Immunisations and vaccination qualified.

Desirable

  • Chronic disease management qualifications.
  • Non medical prescribing qualification

Experience

Essential

  • Experience of working in a Practice Nurse team in General Practice
