Job summary

A Practice Nurse at Molebridge Practice will provide a caring and friendly service for the patients of Leatherhead and Fetcham, assessing and reviewing chronic diseases, managing population health with the provision of vaccines and administering treatment within the scope of their practice and under patient specific and patient group direction.

Main duties of the job

The role will entail:

-Provision of immunisations for babies and children

-Chronic condition management (reviews of asthma/COPD or diabetes)

-Complex dressing and wound management

-Smear taking and smear takers register including independent recall for

-Therapeutic procedures, such as testosterone or denosumab inections

-Travel advice and NHS Travel immunisations

-Infection Control Compliance Responsibilities, including annual audits

-Supervision for other members of the allied health team

About us

The Molebridge Practice is a friendly, local practice that serves 6000 patients across two sites in Mole Valley. One in Leatherhead and another in Fetcham. We have 4 GPs and a multidisciplinary team including ANPs, First Contact Physios and Pharmacists.

Job responsibilities

Job Summary

Molebridge Practice is seeking an enthusiastic and compassionate Practice Nurse to join our friendly multidisciplinary team. The successful candidate will provide high-quality, patient-centred nursing care to our registered population across our Leatherhead and Fetcham sites.

The role involves assessing, monitoring and managing patients with long-term conditions, delivering immunisation programmes, providing preventative healthcare, and administering treatments within the scope of professional practice under Patient Specific Directions (PSDs) and Patient Group Directions (PGDs). The post holder will play a key role in promoting health and wellbeing while delivering safe, effective and evidence-based care.

Main Duties and Responsibilities

The Practice Nurse will be responsible for:

  • Providing childhood immunisations in accordance with the national vaccination schedule.
  • Delivering chronic disease management clinics, including reviews for patients with asthma, COPD and diabetes.
  • Undertaking complex wound care, dressings and ongoing wound management.
  • Performing cervical cytology (smear tests) and maintaining registration on the NHS Cervical Sample Taker Register, including managing independent recall where appropriate.
  • Administering therapeutic injections, including testosterone and denosumab, in line with clinical guidance.
  • Providing travel health advice and administering NHS-funded travel vaccinations.
  • Supporting infection prevention and control standards, including completing annual infection control audits and ensuring compliance with practice policies.
  • Supervising, supporting and acting as a clinical resource for members of the wider multidisciplinary and allied health team where appropriate.
  • Promoting health education, disease prevention and self-management to patients.
  • Maintaining accurate, timely and confidential clinical records using the practice's clinical system.
  • Working collaboratively with GPs and the wider multidisciplinary team to ensure the delivery of safe, effective and high-quality patient care.
  • Participating in clinical governance, audit, mandatory training and continuing professional development to maintain professional competence.
Person SpecificationEssential
  • Registered Nurse with current NMC registration.
  • Experience working within primary care or general practice nursing.
  • Competence in chronic disease management.
  • Qualified and experienced in childhood and adult immunisations.
  • Cervical cytology (smear) trained and registered.
  • Competence in wound care and dressings.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to delivering high standards of patient care and maintaining professional development.
Desirable
  • Experience providing travel health advice and vaccinations.
  • Experience administering therapeutic injections such as testosterone and denosumab.
  • Knowledge of Quality and Outcomes Framework (QOF) requirements.
  • Experience of infection prevention and control audits.
  • Experience supervising or mentoring colleagues.
