Job summary

We are looking for an experienced Primary Care Practice Nurse.

You will be employed as a Practice Nurse on a full time permanent contract. You will be working in our busy GP practice in the Castleford area with a list size of over 11,000 patients.

Main duties of the job

We are looking to recruit a qualified Practice Nurse. Own transport is highly recommended for this post.Hours of work are 37.5 hours. days and times to be agreed.

You will help to deliver nursing services to our local population.

Management of Chronic Disease:

  • Identifying significant abnormalities
  • Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals

Patient health checks

  • Identifying significant abnormalities
  • Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues

Risk assessment

  • Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems)

Health screening

  • Recognising issues and gathering sufficient information to refer (e.g. womens and mens health, sexual health, older people)
  • Undertaking monitoring tasks (e.g. smears) and providing advice as appropriate
  • Providing in-depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans.

Travel health

Immunisation (adult and child)

  • Administering appropriate immunisation autonomously

Our practice has a very strong training ethos. We train GP Registrars and medical students.

Job responsibilities

The Practice Nurse will work as part of the primary healthcare team providing Personal Medical Services to the patients of practice.

Job responsibilities:

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  1. To be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manager
  1. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice
  1. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
  1. To develop, implement and embed health promotion and wellbeing programmes
  1. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
  1. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
  1. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
  1. As required, to diagnose and manage acute and chronic conditions
  1. Provide wound care (ulcer/Doppler, etc.) to patients
  1. To review patient medications to enhance compliance
  1. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages
  1. To co-ordinate the provision of travel medicine services
  1. To request pathology services and process pathology results as required
  1. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  1. To maintain accurate clinical records in conjunction with extant legislation
  1. To ensure SNOMED CT codes are used effectively by all members of the nursing team
  1. To ensure the appropriate maintenance and use of chronic disease registers
  1. To develop, implement, embed and participate in well woman clinics
  1. To develop, implement, embed and participate in well man clinic
  1. To assist GPs with minor surgery when required
  1. To prioritise health issues and intervene appropriately
  1. To support the team in dealing with clinical emergencies
  1. To recognise, assess and refer patients presenting with mental health needs
  1. To implement vaccination programmes for adults and children
  1. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
  1. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  1. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  1. To support the clinical team with all safeguarding matters, in accordance with local and national policies
  1. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

  1. To deliver opportunistic health promotion where appropriate

  • Previous Primary Care experience

  • SystmOne Experience

Qualifications

  • Registered Nurse
  • Experience in Primary Care
  • Qualified in Chronic disease management (diabetes, COPD, Asthma)

Desirable

  • Own transport/access to a vehicle
  • Provision of primary care nursing services within the scope of practice (smears, immunisations, injections, dressings, contraception reviews)
