Practice Pharmacist - GP Surgery
Job summary
We are looking for an enthusiastic and patient-focused Practice Pharmacist to join our team within a busy dispensing GP practice.
Main duties of the job
This is an exciting opportunity for a pharmacist who wants to develop their career within general practice and play a key role in delivering safe, effective and evidence-based medicines optimisation for our patients.
Working as part of the wider multidisciplinary team, you will provide clinical pharmacy support across the practice, working closely with GPs, nurses, dispensary colleagues and other healthcare professionals.

Job responsibilities
Your responsibilities will include, but are not limited to:
- Conducting structured medication reviews and supporting patients with their medicines.
- Reviewing and processing repeat medication requests, ensuring prescribing is safe, appropriate and cost-effective.
- Responding to dispensary and medication-related queries from patients and practice staff.
- Providing advice on medicines, side effects, interactions, adherence and appropriate use.
- Supporting medicines optimisation and promoting evidence-based prescribing.
- Identifying opportunities to improve prescribing quality, safety and value.
- Supporting the management of high-risk medicines and appropriate monitoring.
- Undertaking clinical and prescribing audits and supporting quality improvement initiatives.
- Reviewing medication following hospital discharge, outpatient appointments and changes in clinical circumstances.
- Supporting the practice with implementation of relevant prescribing guidance and recommendations.
- Working collaboratively with the practice dispensary team to ensure safe and efficient medicines management.
- Providing appropriate patient education and supporting shared decision-making.
- Contributing to clinical governance, patient safety and practice improvement.
- Undertaking any other appropriate duties within the scope of the role.
Person specification
Qualifications
Essential
- A GPhC-registered pharmacist with a current licence to practise.
- Experienced in, or keen to develop their career within, general practice/primary care.
- Clinically confident with excellent attention to detail.
- Able to work independently while also contributing effectively as part of a multidisciplinary team
- A strong communicator who can build positive relationships with patients and colleagues.
- Organised, proactive and able to prioritise a varied workload.
- Committed to delivering high standards of patient care and medicines safety.
- Comfortable working within a dispensing practice environment.
Desirable
- Previous experience in general practice, primary care or a dispensing environment would be advantageous, but we would also welcome applications from pharmacists looking to develop their primary care experience.
