RECONNECT Worker- Norfolk - Norfolk

Are you passionate about making a real difference to people rebuilding their lives after prison?

We're looking for a compassionate and motivated RECONNECT Practitioner to support individuals before and after release, helping them access the health, wellbeing and community services they need to successfully reintegrate.

What you'll be doing

  • Deliver personalised support to people preparing for release and for up to six months afterwards.
  • Complete holistic needs assessments and develop tailored support plans.
  • Build strong relationships with prisons, healthcare providers and community organisations.
  • Connect service users with GPs, mental health services, wellbeing support and local agencies.
  • Manage referrals, maintain accurate records and work collaboratively to achieve positive outcomes.

What we're looking for

  • A caring, organised and proactive individual with excellent communication skills.
  • Someone who can build positive relationships with service users and partner agencies.
  • Experience supporting vulnerable people or working in health, social care, criminal justice or a related field is desirable.
  • The ability to manage a varied caseload and work independently while contributing to a supportive team.
  • A valid Full UK Driving Licence and access to your own vehicle is essential.
