Job summary
We are seeking a motivated, compassionate, and skilled Registered Nurse to join our team, working alongside an experienced Community Matron to provide high-quality, holistic care to patients within their own homes.
Main duties of the job
This is an exciting opportunity for a nurse who enjoys autonomous practice while being part of a supportive multidisciplinary team. The successful candidate will be responsible for undertaking comprehensive assessments, delivering evidence-based nursing care, and supporting patients to achieve the best possible health outcomes whilst maintaining independence in their own homes.

Job responsibilities
Deliver high-quality, patient-centred nursing care within the community setting.
Undertake comprehensive holistic assessments and develop, implement, and evaluate care plans.
Utilise clinical examination and assessment skills to identify patient needs and support clinical decision-making.
Work collaboratively with the Community Matron and wider multidisciplinary team to coordinate and optimise patient care.
Monitor and manage patients with complex and long-term conditions.
Promote health education, prevention, self-management, and wellbeing.
Maintain accurate and timely clinical documentation and records.
Communicate effectively with patients, carers, colleagues, and partner agencies.
Identify safeguarding concerns relating to children and adults at risk and take appropriate action in accordance with local and national safeguarding policies and procedures.
Escalate concerns appropriately and contribute to safeguarding investigations and multidisciplinary discussions where required.

Qualifications
Essential
- Registered Nurse with current NMC registration.
- Demonstrable clinical assessment and examination skills.
- Ability to work confidently and independently, managing a diverse caseload within the community.
- Excellent communication and interpersonal skills.
- Strong organisational and time management skills.
- Ability to build positive relationships with patients, carers, and colleagues.
- Understanding of safeguarding responsibilities and the ability to recognise, escalate, and manage safeguarding concerns appropriately.
- Full UK driving licence and access to a vehicle for community visits.
Desirable
- Independent Nurse Prescriber qualification, but not essential
- Previous experience in community nursing or managing patients with complex health needs.
- Experience of multidisciplinary working across health and social care settings.
