Braemar Medical is currently working with one of our valued GP Surgery clients to recruit a Salaried General Practitioner with a special interest in Care Home Medicine.

The Role

As a Salaried GP with a special interest in Care Home Healthcare, you will play a key role in delivering high-quality, patient-centred medical care to residents across the practice's assigned care homes.

Working alongside experienced clinicians, nursing staff and care home teams, you will help improve patient outcomes while ensuring residents receive compassionate, proactive and holistic care.

Key Responsibilities

  • Conduct weekly ward rounds within assigned care homes.
  • Assess, diagnose and manage acute and chronic medical conditions.
  • Complete comprehensive medication reviews.
  • Develop and review personalised care plans.
  • Complete Advance Care Plans and ReSPECT forms.
  • Initiate anticipatory medications where clinically appropriate.
  • Complete Medical Certificates of Cause of Death (MCCD) and refer to the Medical Examiner following expected deaths.
  • Work collaboratively with care home staff, district nurses and the wider multidisciplinary team.
  • Provide clinical advice and support to residents, families and carers.
  • Maintain accurate and contemporaneous clinical records.
  • Deliver safe, evidence-based, patient-centred care in accordance with GMC and NHS guidance.

Person Specification Essential

  • Fully qualified General Practitioner
  • Full GMC Registration with Licence to Practise
  • Inclusion on the NHS England Performers ListEligible to work independently within UK General Practice
  • Excellent communication and interpersonal skills
  • Strong clinical decision-making abilities
  • Commitment to delivering compassionate, patient-centred care
  • Ability to work effectively within a multidisciplinary team
  • Desirable
  • Previous experience working with Care Home residents.
  • Interest in Geriatric Medicine or Frailty Care.
  • Experience completing Advance Care Plans and ReSPECT documentation.
  • Experience undertaking medication reviews.
  • Knowledge of end-of-life care pathways.
