We are seeking an experienced and dedicated Senior Mental Health Practitioner to join our dynamic team within the Cornwall Partnership NHS Foundation Trust Mental Health Service. About You

You must hold a Registered professional qualification (e.g., RMN, Social Worker, Occupational Therapist).

You must have significant experience working in mental health services.

You must have strong communication skills, good organisational skills along with the ability to work autonomously and collaboratively across multi-disciplinary teams.

Main duties of the job

To manage the effective and safe oversight of the allocations and transitions caseload

To conduct reviews of individuals on the caseload, including the current mental health needs of the clients

To maintain and update essential documentation, including Proms.

To provide signposting to appropriate services, including NHS services, waiting well resources, third sector and VCSE offers

To support the safe and efficient movement through services and access to treatment

This is a community-based role that may require frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements
