Social Worker (with 2 years +) (Adults) - Level 3

Social Worker – PO2 Lambeth | £32.66 per hour (Umbrella) We are seeking an experienced Social Worker to join a Lambeth Adult Social Care team. This is an excellent opportunity for a skilled practitioner to make a positive difference to the lives of adults with care and support needs and their carers. Key Responsibilities * Manage a caseload of adults with care and support needs and carers. * Complete Care Act assessments, reviews and reassessments. * Develop and implement person-centred care and support plans. * Undertake Adult Safeguarding enquiries and develop appropriate safeguarding plans. * Apply the Mental Capacity Act 2005, including capacity and best-interest assessments. * Work effectively with complex and challenging cases involving risk, conflict, multiple needs and multi-agency involvement. * Coordinate with partner agencies and professionals to achieve positive outcomes. * Maintain accurate records and produce reports for panels, reviews and legal proceedings. * Provide information and advice around services, welfare rights and financial implications. * Work in accordance with relevant legislation, policies, professional standards and the Care Act 2014. About You You will be a qualified and experienced Social Worker with strong knowledge of adult social care, safeguarding, the Care Act and Mental Capacity Act. You will demonstrate sound professional judgement, effective risk management, relationship-based practice and the ability to work independently with complex cases.
