Job summaryThe Trust is seeking a Specialty Doctor to join the friendly and experienced South Wiltshire Dementia Service. This is an established post which has become vacant due to the current incumbent leaving. It is a great opportunity to gain a great deal of experience and knowledge working within an established multi-disciplinary team. Working with a team who believe in collaborative work and have the patient at the centre of their practise. The service provides a secondary assessment facility and a monitoring service for those who require maintenance prescribing on memory enhancers that cannot be prescribed within primary care. This post will provide support and clinical assistance to the Consultant Psychiatrist concerning the care of patients. Salisbury has a weekly Academic programme. In addition, there is a monthly PDP group and Peer Supervision. CPD activities are supported. The post offers study leave arrangements as well as good research, service development and leadership opportunities. The post-holder will be well supported by Consultant Psychiatrist, Team Manager and Community Services Manager.Please refer to the accompanying job description for full details on the roles and responsibilities of this post.Main duties of the jobThe post holder will support the Consultant Psychiatrist in delivering high-quality, patient-centred mental health care across community and clinic settings.Key responsibilities include undertaking routine and urgent psychiatric assessments, developing and reviewing treatment plans, and contributing to the management of complex cases within the multidisciplinary team.The successful candidate will arrange and participate in Care Programme Approach (CPA) meetings, ensuring service users and carers are fully involved in care planning.They will maintain accurate, timely and comprehensive clinical records using the Trust's RiO electronic patient record system, documenting assessments, clinical decisions and management plans.The post holder will work closely with GPs regarding physical health monitoring and investigation results, liaise effectively with NHS and partner organisations, provide specialist advice to multidisciplinary colleagues, and actively participate in regular MDT meetings to support safe, effective and coordinated patient care.About usWe are AWP (Avon and Wiltshire Mental Health Partnership NHS Trust) a diverse organisation with over 5,000 dedicated staff providing inpatient and community-based mental health care.
