Job summary
Melrose Medical Centre is requiring a record coder and summariser to work in a small but busy GP practice for 16 hours a week. You will be responsible for reviewing patient medical histories, incoming correspondence, and new patient records, extracting key clinical data and translating it into standardized SNOWMED CT codes within SystmOne
Main duties of the job
Core Responsibilities
- Note Summarisation: Review new patient records, historic paper notes (Lloyd George files), or GP2GP transfers to build a clear, chronological summary of past medical history.
- Clinical Coding: Assign accurate diagnostic, procedural, allergy, and medication codes using approved terminology systems.
- Document Workflow: Process incoming discharge summaries, clinic letters, and lab results, flagging urgent items for GPs and filing routine information.

Melrose Medical Centre is a small but busy GP practice in Billingham. There are three Salaried part time GP's, a Practice Nurse, Practice Manager and four administrators

Essential
- Able to input data accurately and efficiently
- Undertake repetitive tasks whilst maintaining attention to detail
- Good communication and interpersonal skills, both verbal and written, face-to-face and non-facing customers
- Demonstrates good customer care skills at all times
- Good working knowledge of office procedures
- Good organisational and planning skills using own initiative
Desirable
- Knowledge of working in an NHS environment
- Ability to maintain and update patient information database(s).
