Support Worker

Job Purpose

To provide person-centred care and support to individuals, helping them maintain their dignity, independence, wellbeing and quality of life while ensuring their safety and comfort.

Key Responsibilities

• Provide compassionate, person-centred support based on each individual's care plan.
• Assist with daily living activities and encourage independence wherever possible.
• Support with personal care, including washing, dressing, toileting and continence care, where required.
• Assist with mobility, transfers and use of appropriate moving and handling equipment.
• Support individuals with meals, drinks and maintaining good nutrition and hydration.
• Provide companionship and emotional support.
• Support individuals to attend appointments, activities and access the community where required.
• Encourage participation in hobbies, social activities and meaningful activities.
• Support individuals with dementia, learning disabilities, physical disabilities, mental health needs and other support requirements.
• Observe and report changes in physical health, behaviour, mood or wellbeing.
• Recognise and report safeguarding concerns, falls, pressure damage, accidents or other risks.
• Follow care plans, risk assessments, medication procedures and safeguarding policies.
