Work Well is a countywide programme supporting people with health-related barriers to find work, stay in work, or return to employment. It is coordinated by General Practice Alliance (GPA), as the lead provider, in partnership with the other VCSE partners. By bringing together healthcare, community support and employment guidance, Work Well offers a joined-up service that helps people access the right support at the right time.

We're looking for full-time Work & Health Coaches to join our friendly, person-centred team on a fixed term contract until March 2029. In this role, you'll work one-to-one with individuals to explore barriers, build confidence, and create practical plans that support improved wellbeing and sustainable employment.

While a full time working pattern is desired, we welcome applications from candidates seeking part-time, starting at 22.5 hours per week, or other flexible working arrangements, and will consider these in line with service needs.

Main duties of the job

As a Work & Health Coach, you'll support people referred into the Work Well programme through personalised, person-centred coaching. You'll help individuals understand their options, navigate local services and take practical steps towards improving their health, wellbeing and employment outcomes.

Working closely with employers, GPs and partner organisations, you'll ensure support is coordinated and effective. You'll also contribute to workshops, maintain accurate records and promote the programme so people across the county can access the service.

Job responsibilities

  • Manage a caseload of individuals referred into the Work Well programme, providing personalised support throughout their journey.
  • Undertake holistic assessments and develop personalised action plans, reviewing progress regularly to help individuals achieve their health, wellbeing and employment goals.
  • Provide information, advice and practical support relating to employment, training, benefits, health and community services, including job search, CV development and interview preparation.
  • Deliver person-centred coaching that builds confidence, resilience, motivation, self-management and health literacy.
  • Work collaboratively with healthcare professionals, employers and partner organisations to coordinate support and facilitate referrals to appropriate services.
  • Help individuals access local services and navigate health, social care and employment pathways.
  • Deliver group sessions, workshops and awareness activities where required.
  • Maintain accurate records, case notes and outcome data using relevant systems.
  • Ensure programme eligibility criteria, safeguarding requirements and organisational policies are followed.
  • Contribute to service evaluation, quality improvement and achievement of programme outcomes.
  • Promote the Work Well service with referral partners and stakeholders to maximise awareness and appropriate referrals.

Skills and Knowledge

Essential

  • An understanding of the relationship between health, wellbeing and employment and the factors that can affect an individuals ability to work.
  • Ability to undertake vocational assessments that identify work aspirations, employment goals, transferable skills, barriers and support needs, enabling the development of personalised plans to support individuals into, within, or back to work.
  • Good problem-solving skills, with the ability to understand individual needs, identify appropriate support and maintain accurate records and action plans.
  • Strong relationship-building skills, with the ability to work collaboratively with healthcare professionals, employers, community organisations and partners to deliver person-centred, coordinated support.
  • Skilled in communicating complex information and managing sensitive conversations with individuals, employers and partners, using negotiation, persuasion and influencing skills where appropriate.
Job Circumstances

Essential

  • Able to travel across Northamptonshire to meet the requirements of the role.
  • Eligible to work in the UK and able to successfully complete required pre-employment checks, including a DBS check.
Qualifications

Essential

  • Coaching/counselling qualification or other relevant qualification or experience involving reflective listening skills, information, advice and guidance.
  • Good standard of education, including literacy and numeracy skills sufficient to maintain accurate records, communicate effectively and manage case information.
Experience

Essential

  • Experience of supporting behaviour change using coaching approaches, frameworks, models or other helping strategies (e.g. Motivational Interviewing, Wellbeing Star)
  • Experience of supporting individuals with physical or mental health conditions, disabilities or other personal circumstances that may affect their ability to gain, sustain or return to employment.
  • Good IT skills and the ability to use digital systems to maintain accurate case records and manage information effectively.
