Job summary

Actively contribute to the multidisciplinary team to deliver high-quality, patient-centred care. Communicate sensitive and complex information with professionalism, empathy, tact, and diplomacy, supporting patients, families, and carers, particularly when discussing difficult or end-of-life situations. Identify and overcome communication barriers through appropriate resources, including interpreters and written or visual aids.Use effective negotiation, persuasion, and de-escalation skills to manage challenging situations, promote treatment compliance, and address conduct or performance concerns. Build strong working relationships across the multidisciplinary team to support safe, effective care and ensure clear patient handovers between healthcare professionals.Support and supervise students on placement, providing clear guidance and feedback. Collaborate with healthcare professionals to prioritise care, manage complex situations, and influence positive outcomes for patients. Maintain robust communication processes within and outside the team, contributing to or leading team meetings, ensuring key information, actions, and updates are effectively shared and implemented

Main duties of the job

The UCR/VW team aims to deliver holistic assessments and interventions to individuals who are at risk of hospital admission within their own home within 2 hours of referral to the service. The aim of the team is to enable people to stay at home, recover, and re-engage with the important aspects of their life and prevent admission to acute sectors

The post holder will work in the Urgent community response team and Virtual Wards to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service.

The post holder will ensure access to Urgent Health Assessments, provide and monitor short term treatment programmes at home and ensure safe discharge or referral to long term health and social care interventions. Utilising clinical expertise, clinical judgement, discretion and decision making in the development of individualised treatment.

To work in partnership with primary, secondary and other health and social care providers, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.

To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.

The post holder will be expected to work within a flexible working pattern over a 7 day week 24/7 in the urgent community response team as required.

Qualifications

Essential

  • *Evidence of professional and clinical knowledge acquired through training to degree/diploma level leading to Professional registration on the appropriate part of the HCPC / NMC register i.e. (Adult / Child / Learning Disability or Mental Health)
  • *Evidence of undergraduate education for newly qualified nurses / or post graduate continuing professional development for registered nurses who have post registration experience

Desirable

  • *Health care management / leadership qualification
  • *Mentorship / assessor training qualification
Experience

Essential

  • *Substantial post registration experience working in a complex health care setting / Acute NHS care setting supervising / mentoring junior staff
  • *Evidence of professional /clinical knowledge in area supplemented by specialist clinical and or managerial training and CPD
  • *Can demonstrate understanding / experience / willingness to be a practice-based supervisor / assessor as detailed in the Nursing & Midwifery Council ( NMC ) standards for student supervision and assessment
  • *Professional portfolio which demonstrates continuing professional development, evidence of ability to maintain professional registration
  • *Relevant and recent clinical experience within the speciality /acute care setting, including the supervision /education of junior staff
  • *Experience of mentoring undergraduate students
  • *Evidence of ability to lead /coordinate a shift /area of department
  • *Experience in learning from incidents /complaints and supporting changes in practice.
