We are seeking a dedicated and committed Children and Family Worker to join our Through Care Service, working specifically within the Pathways Programme. This role focuses on supporting children and young people as they transition from care to independence, and specifically supports them to develop a peer network and access support via direct and group work techniques. You will work closely with partner agencies to deliver practical, emotional, and relational support tailored to individual plans for children in care.The Pathways Programme supports children in care to navigate key transitions such as housing, education, employment, health, and relationships.
