We are seeking an enthusiastic Community Cardiac Rehabilitation Physiotherapist to join our established and supportive Cardiac Rehabilitation Team. This is an exciting opportunity to lead and deliver high-quality, evidence-based cardiac rehabilitation programmes, supporting patients to improve their health, wellbeing and quality of life following a cardiac event or diagnosis.
As an autonomous practitioner, you will manage your own caseload of patients with a range of cardiac conditions, providing specialist assessment, treatment, exercise prescription, education and ongoing support. Working alongside a multidisciplinary team, you will play a key role in helping patients achieve their rehabilitation goals and develop the confidence and skills to manage their long-term health.
Main duties of the job
Hold responsibility for the assessment, planning, implementation and evaluation of cardiac rehabilitation interventions for patients with a variety of cardiac conditions.Lead and deliver cardiac rehabilitation exercise programmes in both community and clinical settings.Support the provision and continued development of a high-quality, patient-focused and evidence-based cardiac rehabilitation service.Work in line with the British Association for Cardiovascular Prevention and Rehabilitation (BACPR) Standards and Core Components.Collaborate with colleagues across the multidisciplinary team to optimise patient outcomes and promote health and wellbeing.Provide education, advice and support to patients and their families, encouraging self-management and informed decision-making.Adopt a flexible, professional and dynamic approach to service delivery, supporting the wider cardiac rehabilitation service as required.About us

Education and Qualifications
Essential
- Relevant degree in Physiotherapy with HCPC registration Or Degree in Sports, Exercise Science or Exercise Physiology Or Level 4 Cardiac Disease (Rehab) qualification in exercise fitness e.g. BACPR Exercise Instructor Training qualification and with current REPs at Level 4
Desirable
- Teaching qualification
Experience
Essential
- Experience of undertaking exercise assessments, treatment, and planning of exercise programmes
- Experience of delivering exercise / physical activity advice/education regarding lifestyle changes to a clinical population
- Prescribing and leading individual and group exercise sessions to patients, who may have multiple co-morbidities. To have the knowledge and skills to be able to adapt patients exercise programmes to enable patients to participate in cardiac rehabilitation safely and effectively.

Knowledge and Abilities
Essential
- Cardiovascular anatomy, physiology, investigations/interventions, and medications and how this influences the principles of exercise testing/prescription for cardiovascular training within cardiac rehabilitation. Coronary heart disease and heart failure (including signs and symptoms and recognition of progression of disease/decompensation) and its implications for risk stratification and exercise programming.
- The range of cardiac conditions and co-morbidities encountered on a typical cardiac rehabilitation programme and how this affects treatment, exercise capacity/ability, adaptations, and contraindications to exercise.
