Community Night Care Practitioner

Job summary

At Compton Care, our Community Night Care Practitioners provide calm, skilled overnight care that enables people with lifelimiting illness to remain safely at home, while giving families vital respite when they need it most. Working independently overnight, youll play a key role in our community rapid response and virtual ward model, helping prevent avoidable hospital admissions and supporting hospital avoidance across our system.

Main duties of the job

You will be there to provide compassionate overnight care and reassurance in patients homes or remotely. To Support people on our virtual ward and urgent community pathways. To Work autonomously, with clear clinical support and escalation routes and to enable families to rest, knowing their loved one is safe

Compton Care provides high quality, accessible care and support for the people in our communities living with life limiting conditions across Wolverhampton, the Black Country, South Staffordshire and East Shropshire. The charitys annual running costs are £16 million, 70% of this running cost is funded through our stores, fundraising, and donations from our amazing supporters.
